How It Works

1. Complete enrollment and in-take forms: Help us get to know you and your needs by sharing your medical and family history, your daily challenges and goals, will enable Ilant to create a plan personalized for you.

2. Meet with your Peer Navigator:
Our Peer Navigators have all lived experience and can provide you ongoing support throughout your journey.

3. Meet with your clinical team:
This includes board-certified obesity medicine physicians, and licensed specialists in nutrition and mental health.

4. Regularly engage and see outcomes:
Meet with your care team regularly and use our app and nurse line between visits for extra support.

FAQs

What does Ilant's program include?

For Zoetis colleagues and dependents 18 years or older enrolled in the UHC health plan, the following are available to you at no cost as part of Ilant's program: regular visits with your clinical team and peer navigator, a connected scale or blood pressure monitor, and access to Ilant's App and educational content. Any labs ordered, medications prescribed, or surgical procedures referred to are not included in Ilant's program but are subject to your health plan coverage, deductibles and co-pays.

How is Ilant Health different from other programs?
Ilant Health is an evidence-based, medical solution that supports you in every step of your cardiometabolic journey. We prescribe FDA-approved medications, as appropriate, along with personalized support from board-certified specialized physicians, licensed mental health clinicians, registered dietitians, and peer navigators. You’ll have regular video visits with your care team, and support through messaging and access to educational tools through the Ilant Member App.
Can Ilant physicians prescribe medications?
Yes - Ilant physicians are able to prescribe all medications relevant to your cardiometabolic health - that may include medications for diabetes, high blood pressure or obesity.  If you and your Ilant doctor decide to pursue anti-obesity medication as a treatment, you may be prescribed medications such as GLP-1s (Zepbound, Wegovy) as well as oral anti-obesity medications.  As a preferred prescriber, Ilant physicians are able provide medically necessary weight loss medications without needing a prior authorization.  Members are not guaranteed a prescription to medication unless it is determined a necessary part of their treatment plan by their Ilant physician.
Who is eligible to participate?

Ilant's program is available as a covered benefit to those Zoetis colleagues and dependents enrolled in Zoetis's UHC health plan and who are 18 years old or above.

What conditions can weight loss help with?

Weight loss can help reduce or even drive remission of the following conditions: Diabetes, Sleep Apnea, High Blood Pressure, High Cholesterol, Heart Disease, and Joint Pain.

What kind of results can I expect?

Ilant members with obesity, on average, see weight loss of 15% after 12 months on program. Individuals with elevated A1C levels, on average, see nearly a 50% reduction in elevated metrics and many members achieve normal A1C, blood pressure, LDL, and Triglycerides. Many Ilant members have been able to stop certain medications as these conditions improve.  In addition to their health benefits, Ilant members report increased energy and the ability to engage in new activities.

What conditions does Ilant Health treat?

Ilant Health offers eligible Zoetis colleagues and dependents four programs. For those with a BMI under 27, you may be eligible for our Prevention Program that supports members in developing healthy nutrition and behavioral habits. For those with a BMI of 27 or above, you may be eligible for our Obesity Management program, which may combine lifestyle changes with a medical treatment such as medication or bariatric surgery. For those with long-term diabetes, we also offer a Diabetes Management program. And for those with a cardiovascular condition, such as hypertension, we offer a Cardiovascular Management program. The ultimate decision on the right program and treatment for you will be made in a discussion between you and our clinicians, taking into account your medical needs and personal preferences.

Is Zoetis covering GLP-1s like Wegovy and Zepbound?

Zoetis covers Wegovy and Zepbound for members enrolled in their health plans. For any health plan members accessing these medications outside of the Ilant program, there is a Prior Authorization process.  

What happens if I’m using Teladoc today?

Teladoc is being discontinued after 2025.  Don't worry about the transition – Ilant will help set you up with their new platform and introduce you to your care team. You may continue to use any devices that you received from Teladoc but will no longer have access to record results in the Teladoc app.  Ilant is able to connect with those devices, or provide new scales and blood pressure monitors for those who need.

Coverage for diabetes supplies previously covered by Teladoc such as test strips, lancets, or smart glucometers is now available at no cost to you through UHC. I have a primary care physician who prescribes my medications; does Ilant make sense for me?

We love that you’re working with a trusted primary care physician, but you may still benefit from Ilant’s expert care including physicians with expertise and board certification in obesity and from the extended care team that includes nutrition, mental health, and peer-based support.  This type of support – as well as the digital tools and monitoring provided by Ilant – have been shown to support better outcomes.  Ilant will work closely with your existing clinical team to make sure we’re coordinated.

I am already on a certain treatment; how will Ilant address that?

We are big believers in continuity of care and taking a careful and deliberate approach.  If you are on a treatment that is working well, we will be excited to support you on that treatment.  If you’re on a treatment that maybe is not working, we’re going to work together to see if we can help make it better – whether that’s reducing side-effects or improving outcomes.
